Before you begin entering your session information:

• You must complete your personal details, recording agreement, and disclosure before you can be sent your confirmation as a presenter. Deadline for completing this information is June 4, 2012, 11.59 p.m. (Pacific).
• All presenters receiving an honorarium must have a W-9 Form on file with ASHP. Submit a new W-9 if you have not submitted one since 6/1/11, have moved, or have changed your name. If in doubt, submit a new one. Download W-9 and attach Cover Sheet. Email to fmdw9@ashp.org or fax to 301-634-5944.
