XMRV and Chronic Fatigue Syndrome: Risk Management of a Known Unknown
Ritchard Cable, MD
Description
Canadian Blood Services (CBS) was the first organization to alter its donor deferral policy for individuals with a history of chronic fatigue syndrome following initial reports of possible blood transmissibility of the associated XMRV virus. Available data on XMRV, the rationale for CBS' change and its impact on their donor base will be reviewed.
The REDS-II RISE study is currently assessing the effects of blood donation intensity on hemoglobin and iron status. Study design for the development of better predictive models for iron depletion/hemoglobin deferral and improved donor screening strategies will be discussed. Information will be provided regarding targeted iron supplementation strategies in blood donors, and improved counseling messages regarding diet and supplements.

LEARNER OUTCOMES:
  • Cite the rationale underlying the change in Canadian donor deferral practices for chronic fatigue syndrome.
  • Describe current information on the pathogenicity of XMRV and its potential transmissibility.
  • Discuss the impact of blood donation on hemoglobin and iron status in donors enrolled in the REDS-II RISE study and how these data could be used to formulate optimal blood donation guidelines and standards.
  • Identify optimal laboratory measures for predicting development of iron depletion and hemoglobin deferral.
