Organizations face difficulties when attempting to move to a collaborative, supportive error management system. During this session we will discuss the challenges inherent in the process, methods that facilitate a widespread problem-solving mentality among staff, and skills that aid supervisory staff in this journey.


  • Define essential organizational elements that support this cultural change.
  • Describe how to have an error management system that is effective without being punitive.
  • Discuss the differences between problems related to procedures and practices and those related to individuals.
