Disclosure of adverse events and appropriate documentation of the disclosure is needed to provide patients the information required to plan for healthcare. The Joint Commission now requires accredited facilities to have a process in place for the disclosure of unanticipated outcomes. Physicians and healthcare providers daily disclose information about diagnoses, prognoses or complications of anticipated complications; however, there is a hesitancy regarding disclosure of adverse events based on fear of malpractice claims. Many healthcare providers who believe in the honesty of disclosure nonetheless are unsure how to proceed and where to document the disclosure. The speaker will explain the what, why, when and how of disclosure and its impact on patient safety.
