Find out how Novant Medical Group of Winston-Salem, North Carolina, created a remarkable 29% reduction in all-cause hospital admissions and reduced adverse drug event admissions to less than 5% of all hospital admissions. This session will describe the successful process that achieved these results, which you can easily replicate in your setting.
