Pre-registration is required; Registration must received on or before November 22.
Registration is limited to 60 participants. Lunch is on your own.

In this workshop, learn to build a transitional care plan with patients in various practice settings such as the emergency department, inpatient perioperative setting, subacute rehabilitation, outpatient pain clinic services, cancer center, and palliative care. Using patient cases, this interactive workshop will focus on complex pain management issues involving persistent pain, comorbid physical conditions, substance abuse, multimodal treatment, opioid dosing and medication safety. Using patient-centered care models demonstrating best practice, evidence-based strategies for pharmacotherapeutic decisions, and utilizing quality assurance measurements for assessment of outcomes.

  • Assess patient to identify comorbid conditions that complicate the management of pain and other symptoms.
  • Design the treatment plan to optimize outcomes and incorporate evidence-based guidelines for medication dosing and side effect monitoring.
  • Develop a model for communicating the pain treatment plan across multiple practice settings including the medical home with involvement of the patient and family.
  • Identify strategies to prevent adverse events from opioid therapy and manage risk associated with controlled substances.
