Description
This session will focus on applying the concepts from the morning session.

  • Define and describe critical elements of culture and its importance to behavioral change inherent to practice redesign.
  • Descibe a process and tools for defining priorities for improvement in a range of care settings across the continuum of care, including assessment, diagnosis, and execution, solidifying will, developing ideas, and executing strategic initiatives for sustained practice change.
  • Frame an approach for launching a comprehensive interdisciplinary system-wide initiative for redesigning medicacation management philosophy, process, and collaboration.
