Description
As pharmacists, we are often asked to lead efforts that ensure compliance with medication management standards and to improve medication safety, as cited in National Quality Forum Safe Practice #18. In your medication-use process, do you know how and where your so-called “Swiss cheese holes” line up? Participate in this session to learn how pharmacists and technicians can proactively reveal weaknesses in medication-use processes using tracer rounds.

  • Describe a comprehensive monthly ward and clinic inspection program, including how to document findings, report problems, and implement solutions in your practice.
  • Describe the components of the National Quality Forum Safe Practice #18, including its emphasis on leadership.
  • Describe the rolls and contributions of members of the multidisciplinary team to initiate and conduct tracer rounds.
  • List the most challenging areas of the medication-use process and mitigating strategies to minimize risk.
