  • Describe the Center for Medicare and Medicaid Innovation - Partnership for Patients core programs and aims to improve patient safety by decreasing hospital acquired conditions and enhancing care coordination through reductions in readmissions.
  • Discuss ASHP efforts to help members reduce adverse drug events and improve care transition initiatives including partnerships to align the measurement of adverse drug events and readmissions.
  • List examples of processes, procedures, and systemic approaches used by ASHP members who have decreased adverse drug events and reduced readmissions in their communities.
