Description
This session will focus on the latest clinical considerations pertaining to anticoagulation in patients with advanced age. Current controversies related to safety and medication use in this population that may negatively affect anticoagulation will be discussed, as will an evidence-supported perspective and case-based approach to anticoagulation management.

  • Apply current evidence and literature findings to the anticoagulation needs of geriatric patients.
  • Formulate a care plan in response to case vignettes highlighting controversial areas of evidence within anticoagulation in geriatric patients.
  • Identify areas of uncertainty within the literature and medication safety issues as they pertain to anticoagulation in the coexistence of poly-pharmacy.
