Learning Objectives:
Given a patient case, identify and appropriately manage clinically significant warfarin-related drug, herbal, and nutrient interaction.
List the key elements of a business-practice plan for an anti-thrombosis management service.
Identify potential payment mechanisms for anti-thrombosis management services.
Compare and contrast the features of the commercially available anticoagulation point-of-care monitoring devices.
In specific patients, develop a pre-procedure plan for stopping warfarin and starting a bridging agent, and a post-procedure plan for restarting warfarin and use of a bridging agent.
Recommend specific methods to minimize the risk of thrombosis associated with discontinuation of warfarin, and the risk of hemorrhage associated with bridge therapy.
