As president of Healthcare Risk and Safety Strategies, Ms. Gaffey utilizes her extensive background in Risk Management and Patient Safety, including her experience in managing in-house risk management, risk financing and claims programs, and as a consultant for a Top-100 broker, to provide tailored solutions to healthcare organizations across the care continuum. She is an author and frequent lecturer, and currently sits on the Board of Directors of ASHRM, and is past president of the Virginia Chapter. She holds a BSN from the University of Virginia, an MSN from George Washington University, holds the CPHRM certification and the FASHRM designation.

Description
As health care enterprises grow and expand to include settings that take patients through many transitions of care, the risk of error and injury is increased. During these transitions, lack of communication can result in redundant or conflicting information that often creates serious issues for patients, their caregivers, and their families. This presentation will identify practical tools and resources that can be used by health care professionals to improve communication in a consistent manner between care settings and reduce risk associated with care transitions.
