David M. Benjamin, a nationally-recognized scholar in Legal Medicine, Reducing Medication Errors, the Drug Development process and Forensic Toxicology. Received a Ph.D. in Pharmacology from the University of Vermont College of Medicine and completed specialty training in Clinical Pharmacology & Toxicology at the University of Kansas Medical Center.
Dr. Benjamin has authored over 200 presentations and publications and 9 textbook chapters. He is a Fellow of numerous professional groups, including ASHRM.
Dr. Benjamin teaches Clinical Pharmacology at Tufts Medical School. He holds adjunct appointments at Boston University School of Medicine and the College of Health Sciences at Northeastern University.

Ms. Rozovsky is President of The Rozovsky Group, Inc. Ms. Rozovsky works with clients along the continuum of care, providing healthcare professionals, organizations and leadership with practical risk management and patient safety solutions. She has lectured extensively and authored or co-authored over five hundred articles and several books including Consent to Treatment:A Practical Guide 4th Ed. and Health Care Organizations Risk Management: Forms, Checklists & Guidelines, Second Edition with Jane Conley. Fay is team leader for Module II in the Barton Certificate Program. In 2008 Providence College awarded her an Honorary Dr. Public Health for her work in risk management.

Description
Claims data offer valuable insights into medication errors. Data indicates that a primary causal factor involves a breakdown in communication with and about patients regarding medication management. This session will examine specific subset communication issues, including those that involve physician-patient, physician-nurse, physician-pharmacy and care provider-familial care giver situations that result in medication errors. The presenters will offer a series of risk resolution strategies that involve patient communication techniques, screening methods, and documentation strategies. Participants will receive a toolkit that encompasses a patient intake medication inventory tool, a medication discharge summary reconciliation checklist, suggested readings, suggested websites, and reading material.
