Anne Flood, MA, BA, RN, is Director of Quality/Risk Management and Patient Safety Officer at Union Memorial Hospital/MedStar Health in Baltimore, Maryland. A long-time member of ASHRM, Flood was instrumental in the development of ASHRMs patient safety program and has been a frequent contributor to the ASHRM Journal. She is also involved in the Maryland Patient Safety Center and RIMS.
Bob Latino has spent 25 years in the field of Reliability Engineering specializing on the application of FMEA, Opportunity Analysis and Root Cause Analysis. He has authored two books on the topic and co-authored two others, as well as published more than 50 articles on the topic in various trade journals. Bob is also a practitioner, educator, consultant and international speaker on the topic. He recently lead a national RCA team for the CMS/Fistula First Breakthrough Initiative (FFBI). This project was in concert with IHIs participation.
Description
Prevention is key to all aspects of healthcare, including patient safety. Case study material on topics like safety issues related to intra-hospital transfers will reveal how miscommunication and process inconsistencies compromise patient safety, and how silos within hospitals exacerbate the challenges. Recurrences can be eliminated and improvements sustained when organizations utilize RCA beyond satisfying regulatory requirements. Proactive RCA helps providers to identify and prevent the 20% of events that result in 80% of the annual losses. A tool kit will provide practical materials for implementing RCA to proactively eliminate undesirable outcomes, as well as improve processes and reduce costs.
