Larry Veltman, M.D. practiced obstetrics and gynecology in Portland, Oregon. He was the Chairman of the Department of Obstetrics and Gynecology at Providence St. Vincent Medical Center. He chaired the Professional Liability Committee and serves on the Committee on Patient Safety and Quality Improvement of the American College of Obstetricians and Gynecologists. In 2000, he chaired the ASHRM Task Force that wrote “Risk Management Pearls for Obstetrics.” He has published articles and given presentations dealing with medical malpractice, improving communication for the perinatal team, system failures and adverse obstetrical outcomes, and disruptive behavior as a patient safety threat.

Description
When a perinatal injury to a mother or newborn occurs, it is critical for the risk manager to take certain steps anticipating potential litigation. Three checklists are presented to assist in this preparation: 1) a general checklist that addresses potential systemic issues that may have contributed to the event, 2) a checklist for the perinatal team to capture critical information in the immediate aftermath of the event, and 3) a checklist that can be implemented to help prevent perinatal injuries in the future. Recently introduced guidelines, the medical literature, and case review will be used to verify the checklists.
