Ms. Connolly has serve as faculty for several Premier Inc. and Institute for Healthcare Improvement collaboratives. She helped in the development and initial testing of an obstetric trigger tool with IHI. Currently she is working to promote the membership of a Patient Safety Organization with ECRI Institute and as project coordinator with a national perinatal safety collaborative. Ms. Connolly has over twenty years of experience in various healthcare risk management positions. Ms. Rozovsky is President of The Rozovsky Group, Inc. Ms. Rozovsky works with clients along the continuum of care, providing healthcare professionals, organizations and leadership with practical risk management and patient safety solutions. She has lectured extensively and authored or co-authored over five hundred articles and several books including Consent to Treatment:A Practical Guide 4th Ed. and Health Care Organizations Risk Management: Forms, Checklists & Guidelines, Second Edition with Jane Conley. Fay is team leader for Module II in the Barton Certificate Program. In 2008 Providence College awarded her an Honorary Dr. Public Health for her work in risk management.

"Trigger tools" alert staff to emerging clinical risks, but may also unleash a variety of risk exposures, including liability claims, regulatory scrutiny, accreditation entanglements and quality reviews by private payers. The speakers will offer strategies to transform trigger tools into useful drivers for patient safety and risk mitigation.
