Description
This session will cover the integration of systemic therapy into the management of squamous cell cancers of the head and neck patient, with an emphasis on the underlying clinical trial basis for these treatments. The use of concurrent chemoradiotherapy will be presented including the choice of agents, dosing schedule and how this type of treatment can be integrated with altered fractionation radiotherapy. The indications for and trials supporting the use of induction chemotherapy will be discussed. The last section will examine patients initially treated with surgery who will require post-operative chemoradiotherapy, as well as the indications and rationale for treatment. In addition, associated side effects and their management will be discussed. At the conclusion of this activity, the learner will be able to do the following: 1. Discuss the indications and supporting evidence for using concurrent chemoradiotherapy in definitive treatment of squamous cell cancer of the head and neck (SCCHN), including appropriate radiation and chemotherapy combinations. 2. Discuss the evidence and indications for using induction chemotherapy, including the most common drug regimens. 3. Discuss the evolution of the use of post operative radiation therapy (PORT) in SCCHN; understand the risk factors indicating a patient should receive PORT, and those factors that identify a patient who is a candidate for treatment intensification, such as chemoradiotherapy.
