This presentation will demonstrate successful engagement of parent partners in the Pennsylvania and Massachusetts Medical Home Programs. The session will review resources and tools for recruiting parents as quality improvement partners. Lessons learned from Pennsylvania and Massachusetts will be shared.
Improving Family-Centered Care Through Self Assessment: A Tool for Families and Providers
This session will present findings from the use of a self assessment tool developed by an expert panel of family members, state and federal policy-makers, providers, and academics. The tool, in family and professional versions, will be disseminated.
