This session will demonstrate lessons from 35 health care teams who engaged patient and family team members in improving health care for, and empowering people with chronic illness, and their families, to better manage their ongoing health conditions.
Journey of Self-Management Support in an Integrated Health System
This session describes how Gundersen Lutheran piloted and spread self-management support in primary care by participating in the New Health Partnerships virtual community, using the model for improvement, and inviting patient advisors as team members with specific examples and tools.
