The Family-Centered Medical Home: Applying Lessons Learned from Pediatrics to Adult Primary Care
For more than a decade, the medical home model has been developed and implemented in pediatric primary care settings. This session will discuss key practices and lessons learned from pediatrics that can facilitate the transfer and adoption of core elements of the family-centered medical home model to adult health systems of care.
