In 2006, a health care network began the journey to “take patient-centered care to the next level.” This presentation details the visioning process, project infrastructure, long range strategic plan, and methods to achieve the desired future state.
Partnering with Families to Integrate Patient- and Family-Centered Care
This session will assist family members and health care providers to successfully integrate patient- and family-centered care throughout their health care organization.
