Assessing Parent/Caregiver Perspectives and Social Networking (Web-Based): The Pennsylvania Experience

The PA Medical Home Program is a quality improvement initiative working with pediatric practices across Pennsylvania in adopting/implementing medical home principles. A comprehensive family/caregiver survey was developed. In addition, a web-based social networking site for parents was developed.

Supporting Children and their Families through Interactive and Innovative Methods

This presentation will highlight some of the initiatives of this constantly evolving website, and its uniqueness in supporting health care professionals as they guide children and their families through the course of their health care.
