For the 2007 Annual Scientific Meeting

Follow these steps to complete your overall meeting evaluation and to request CME/CPE/CEU credits for the sessions you attended:

1. Enter your meeting badge umber, your email address and name
2. Complete the overall meeting evaluation form
3. Identify the education session(s) you attended
Complete your evaluations for the individual sessions attended
4. View your updated live attendance certificates

NOTE: Printers will not be available onsite at the Annual Scientific Meeting. When you return from the meeting, you can log on to the meeting evaluation site anywhere you access to the Internet and a computer to update your attendance records and print out your attendance certificate.
