Description
Although testicular seminoma is the most common and most curable solid malignancy in young men, a 2010 survey study of American radiation oncologists demonstrated both knowledge deficits and barriers to the implementation of evidence-based medicine for patients with seminoma. Our scientific session on seminoma at ASTRO 2010 was specifically designed to address controversies in the management of Stage I seminoma, focusing on the best possible medical evidence. We propose a follow-up scientific session which will focus on 1) new, less intense surveillance protocols, the role of MRI in surveillance, and the potential dangers associated with medical imaging; 2) management of Stage II seminoma focusing on new, evidence-based radiation fields, radiation doses and emerging interest in cisplatin-based chemotherapy (which we oppose for early Stage II disease), and 3) a presentation on the barriers to appropriate up-front care which affect testicular-cancer mortality and all-cause mortality (including suicide), and on evolving strategies in survivorship care to identify patients at risk for early intervention. At the conclusion of this activity, the learner will be able to do the following: 1. Describe less intense surveillance protocols for Stage I seminoma, as well as the role of MRI and the potential risks of diagnostic imaging. 2. Outline new innovative smaller fields and lower doses used for Stage II seminoma, as well as the risks and benefits of cisplatin-based chemotherapy. 3. Describe the socioeconomic and racial impediments to appropriate care for men with seminoma, as well as the medical and psychological ill effects, including suicide in survivors, and will become familiar with evolving strategies to identify and assist patients at risk.
